To do it right (a living will), read the above website, which was the only site I found that was not trying to sell documents for living wills. It has a FAQ and forms you can fill out (you'll need to have them witnessed, and probably notarized--I didn't read that carefully).
I should look into WA forms, I guess. (I think I've discussed funeral-type wishes with people, but I don't know that I've ever discussed living will stuff. It's just not that fun to talk about.)
